Person to personOur clients need care and assistance. But first and foremost, our clients are people who deserve to be treated with respect and dignity. This is why we strive to build a relationship of equality between ourselves and our clients and to adopt a more personal approach, working on a one-to-one basis. Nothing is more personal than care and assistance.
Cordaan fills the gapsCordaan helps where and when necessary, 24hrs a day, 7 days a week. At Cordaan we help our clients to participate in society as a family member, a local resident and a citizen. We are here for the elderly and for people with a physical or learning disability. If necessary, we can provide home care but we also run nursing homes, residential care homes, assisted living facilities, day centres for work and daily activities, and other services. We work in partnership with general practitioners, specialists, hospitals, local authorities, housing associations and community centres, which enable us to devote all our attention to providing the right care to suit our clients' specific needs.
Putting control in the client's hands We draw up a personal care plan for each of our clients based, in so far as possible, on their individual wishes and needs.
Our servicesCordaan provides a broad spectrum of care under the Exceptional Medical Expenses Act (AWBZ) in close proximity to where the client lives. We provide the following care:
We have centres throughout Amsterdam and also in Diemen, Huizen, Maartensdijk and Nieuw Vennep.
Cordaan Home Care provides personal care to people in their homes. We work with care coordinators who customize our services to the individual wishes of our clients. Care coordinators form the essential link between the GP and the client and sit down with each client to draw up an individual care plan. Cordaan Home Care provides a wide range of home care services, from assistance with domestic tasks to specialized nursing or personal care, renting medical equipment and devices, and offering courses and information on health and nutrition.
What else do we offer?Meals on wheels service, alarm response service, equipment lending service, health information, night care assistance, on-call assistance, specialized nursing care, practical home help and assistance, specialized care for children, dietetics and support for informal carers.
Day care for the elderlyOur care centres offer day care provision for the over-55s and their partners. We provide day care in a structured, safe and pleasant environment and offer a varied programme of social and creative activities.
Residential care centre for the elderlyCordaan runs fifteen residential care centres for elderly people who are no longer able to live alone. Residents in these centres are given their own apartment and are offered different forms of care and support according to their needs, such as nursing care, personal care, domestic help and various forms of assistance. All our residential care centres offer facilities such as a restaurant, café, shop, fitness suite, internet café, and physiotherapy and ergotherapy services.
Nursing homesOur four nursing homes provide care for people who require constant nursing for a duration of time. This could be following a hospital discharge, for example, or elderly clients with psychogeriatric complaints. Our team of dedicated staff consists of nursing home physicians, nurses, caregivers, physiotherapists, speech therapists and ergotherapists. All our nursing homes offer the following facilities: restaurant, café, shop, fitness suite, internet café, and physiotherapy and ergotherapy services.
ActivitiesWe organize activities in all our residential care centres, ranging from romantic dinners to a visit to the zoo, bingo nights and keep-fit classes.
For childrenParents of children with a learning disability can come to us for support and assistance. We provide support and advice on parenting problems and, where necessary, we also provide temporary respite. We can organize activities with the children to enable the parents to concentrate on other pressing matters.
Nowadays, an increasing number of children with a learning disability are entering mainstream education. In an effort to promote integration, Cordaan provides support to teachers and auxiliary staff in schools and child care centres. For children who are not able to participate in mainstream education, we have Child Service Centres. These centres offer a variety of disciplines such as physiotherapy, speech therapy, ergotherapy, supervision and music therapy to support the child's development. After-school child care and weekend child care provision is also available.
If, despite all efforts, living at home proves impossible, Cordaan can provide accommodation for children with a disability. The parents are strongly encouraged to take an active part in the supervision.
Assisted independent livingCordaan provides support to adults and young adults living independently in the form of individual flats rented by the client. Or if preferred, clients can rent flat units in an apartment block, which often has a help centre where clients can go for a chat, a coffee or a meal.
Living in a groupThere is also a wide range of options for people who prefer to live in a group and where 24 hour care is provided.
Constant careThree of our residential care facilities in Amsterdam provide constant care, one-on-one where necessary, to people with serious and/or multiple disabilities. Staff are able to make contact with their clients through the use of pictograms, gestures and sound recognition. Our carers provide a safe environment and maintain a regular routine.
Work and daily activitiesCordaan provides adults with an extensive range of work within a protected environment, with activities including gardening, cleaning, light administrative tasks, catering and various arts and crafts. We also second groups to different companies and institutions and, where possible, try to integrate our clients into mainstream work.
For those requiring constant supervision and/or care, Cordaan offers a range of day centres offering our clients opportunities to participate in society. This is done in the form of communal activities in the local neighbourhood, such as going for coffee, cycling or walking. We always remain within reaching distance of the clients' accommodation.
Assisted independent livingPeople with psychiatric or psychosocial problems can rent local accommodation through Cordaan and we provide the supervision.
Sheltered accommodationFor clients who are no longer able to live independently, Cordaan provides supported accommodation in a communal household, which provides personal care and support with everyday living and domestic tasks. Cordaan provides different forms of accommodation, including 24-hour supervision, where a care assistant is also present at night. Some of our other supported accommodation facilities provide only daytime supervision.
Day activitiesOur day centres offer a wide range of activities, from simply dropping in for a coffee to signing up for one of our courses or taking part in work-related activities to prepare you for entering the workforce.
Guidance centreThe guidance centre offers one-to-one counselling in matters regarding work, accommodation, daily activities, learning, and building up a social network.
Informal carers and volunteersInformal carers and volunteers play an essential role in the care of our clients. They provide additional support such as extra personal attention, taking clients for an afternoon's shopping or reading the paper together. Markant, a subsidiary of Cordaan, provides support for informal carers and volunteers in a variety of different ways.
Team workCordaan works closely with other groups and institutions, such as mental health institutions, hospitals, organizations representing the ethnic minorities and patient and parent organizations. Other important partners include community and neighbourhood centres, sport clubs, educational institutes, housing associations and local authorities.
Key figuresCordaan employees 9,000 staff members and 2500 volunteers. Each year, we provide care and assistance to around 30,000 clients.
ContactAll care and assistance services provided by Cordaan and Cordaan Home Care are subject to an indication from the Care Indication Centre (CIZ). If necessary, we are able to mediate on your behalf. Under the terms of the Exceptional Medical Expenses Act (AWBZ) or the Social Development Act (WMO), you are entitled to reimbursement of the majority of these costs.
